Insurance:
Therapy Services (and some limited assessment services) may be covered in full or in part by your health insurance or employee benefit plan. Equilibrium Health currently accepts the following insurances: Aetna, Blue Cross Blue Shield, CIGNA, Johns Hopkins EHP, USFHP, Tricare, Medicaid, and Medicare.
Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible, and has it been met?
- How many visits/sessions per year does my health insurance cover?
- What is the coverage amount per visit/ session?
- Is approval/referral required from my primary care physician?
Payment:
Cash, checks, and all major credit cards are accepted for payment.
Cancellation Policy:
Except for unpredictable emergencies, we will ask you to pay $125 for any missed appointment or appointment canceled within 24 business hours of the scheduled time.
