Client Responsibilities

1. Furnish accurate and complete personal and medical details during the intake process.
2. Attend all scheduled appointments on time and adhere to the cancellation policies.
3. Actively engage in the treatment process, follow Provider recommendations, and communicate any concerns or changes in your condition.
4. Ensure timely payment for services as per the Provider’s fee structure and payment terms.

Provider Responsibilities

1. Deliver professional psychiatric mental health services in alignment with industry standards.
2. Safeguard client privacy and confidentiality in accordance with relevant laws and regulations.
3. Develop a personalized treatment plan based on the client’s specific needs and objectives.
4. Notify clients of any modifications to scheduled appointments or treatment plans promptly.
5. Uphold the client’s rights, autonomy, and cultural values throughout the therapeutic process.

SMS Terms of Service

By opting into SMS from a web form or other medium, you are agreeing to receive SMS messages from Equilibrium Health LLC. This includes SMS messages for appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications. Message frequency varies. Message and data rates may apply. Message HELP for help. Reply STOP to any message to opt out.

By texting Equilibrium Health LLC, you are agreeing to receive Conversations (external) messages from IC Health Services. Reply STOP to opt-out; Reply HELP for support; Message & data rates may apply; You may receive 10 messages per day.

Intellectual Property

1. All materials provided by the Provider, including treatment plans, therapy notes, and educational resources, are protected under intellectual property laws.
2. Clients are prohibited from reproducing, distributing, or altering any materials without prior written permission from the Provider.

Limitations of Liability

1. The Provider’s liability for any claims related to the services rendered is limited to the fees paid by the client for those services.
2. The Provider is not responsible for any indirect, incidental, or consequential damages resulting from the use or inability to use the services.

Governing Law and Jurisdiction
